本人情報ページ。氏名、生年月日、国籍を確認します。Identity page used to confirm name, birth date and nationality.

基礎年金番号が分かる資料Basic pension number evidence

年金手帳、基礎年金番号通知書、その他番号を確認できる資料。Pension book, basic pension number notice or other evidence.

日本での勤務期間が分かる情報Japanese employment period information

会社名、入社日・退職日、同年中の他の退職金の有無など。Employer, start/end dates and whether other retirement payments were received that year.

還付金の受取口座情報Refund bank account information
